The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, most major credit cards, and limited CareCredit. Payment is expected at the time services are performed.  When more extensive dental care is necessary, financial arrangements can be made with our office.   Insurance deductibles, co-insurance and percentage portions are due the day of you visit.   All monthly statements are due and payable in full unless prior arrangements have been made with the office.


 

 
 
Dental Insurance

We are a Delta Dental Premiere provider and do submit claims for many other insurance carriers. Our staff will prepare all the necessary forms for you dental benefits.  We remind you that your specific policy is an arrangement between you and your insurance company. Thus, the insurance company is responsible to the patient, and the patient is responsible to the doctor.  We cannot render services on the assumption that the charges will be paid for by an insurance company.  Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be.  

The fees charged for services rendered to those who are insured are the usual and customary fees charged to all of our patients for similar services.  Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees.  You should be aware that different insurance companies vary greatly in the types of coverage available.  Also, some companies take care of claims promptly while other delay payment for several months.

Cancellation

Your appointment time is reserved exclusively for you.  If you are unable to keep the appointment, please provide us with at least 24 hours notice. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
